0165(2,a): licensor observed in the school age room, object that is peg like and small detach easily from a cubby underneath the television. 0165(2,c): licensor observed a cord in the storage room that is in the hallway of the school age classroom accessible to children. 0165(2,d): licensor observed plastic bags in the pre-k classroom bathroom cubby, underneath an unused sink in the school age classroom, and in the storage room that is in the hallway of the school age classroom. 0165(3,a): licensor observed an item that has a screw still attached underneath an unused sink in the school age classroom. 0165(3,f): licensor observed the following cabinets not secured to the wall: shelving unit in the pre-k kitchen; brown cabinets in the pre-k hallway and bathroom; school age classrooms have a stacked colored drawer white cabinet shelving unit. 0165(3,g): licensor observed play cans in the play kitchen have rust on them and accessible to children. 0165(5,a): licensor observed an electrical outlet in the school age classroom near the television wall exposed without a plate.
